The Medical Certification for Disability Exceptions Evaluation (Form N-648) is designed for individuals applying for U.S. citizenship who are unable to meet the English and/or civics requirements due to physical or mental disabilities. During this evaluation, a licensed clinical psychologist assesses the individual’s condition to determine if their cognitive functioning significantly impairs their ability to learn or demonstrate knowledge of English or U.S. history and government.
